Health Screening Services

At Second Chance Opportunities, we believe everyone deserves the support they need to live a healthy, stable life. As a participating provider in New York State’s Social Care Network (SCN), we offer free Health-Related Social Needs (HRSN) screenings for Medicaid members.

These screenings help us understand what’s going on in your day-to-day life and connect you with services that can make things a little easier.

What Is an HRSN Screening?

An HRSN screening is a short, confidential check-in that helps us learn more about the everyday factors that may be affecting your health.

With your permission, we may ask about:

  • Housing and utilities

  • Food access

  • Transportation

  • Employment and education

  • Interpersonal safety

  • Financial or social stress

Your answers help us determine what kinds of support may be available to you through New York State’s Social Care Network.

How We Support You

We work alongside Healthy Alliance and a network of trusted community partners to make sure you’re connected to the right resources.

We start with a conversation.

If your screening shows that you could benefit from support, we’ll talk through your options together.

We help you get connected.

With your consent, a Social Care Navigator will guide you through the next steps and connect you with local providers.

Services are provided at no cost.

If you’re eligible, the services you receive are completely free and tailored to your needs.

We check in along the way.

Your Care Manager or Healthy Alliance may follow up to ensure everything is working as it should.

Guidance from nutrition professionals; Groceries, fresh produce, or prepared meals; Cooking supplies such as pots, pans, microwaves, or mini refrigerators

Who Is Eligible?

You may qualify if:

  • You are a New York State Medicaid member

  • Your screening shows that you have health-related social needs

Eligibility is confirmed through the screening and a follow-up assessment.

How to Get Started

Getting started is easy and only takes a few minutes. Screenings are always done with your permission, and your information is kept private.

You can:

  • Visit Second Chance Opportunities in person

  • Complete a self-screening online

  • Ask your doctor, health center, or another provider to help you get started
